Chiro CoOp

Chiro Co-Op is a membership program created to enable patients to maintain their musculoskeletal health through regular treatments and periodic check ups. Chiro Co-Op is not insurance and not meant to replace active care for acute or chronic conditions but instead is maintenance care. 

Maintenance therapy is defined by the United States Department of Health and Human Services via CMS as "a treatment plan that seeks to prevent disease, promote health, and prolong and enhance the quality of life; or therapy that is performed to maintain or prevent deterioration of a chronic condition. When further clinical improvement cannot reasonably be expected from continuous ongoing care, and the chiropractic treatment becomes supportive rather than corrective in nature, the treatment is then considered maintenance therapy"

Chiro Co-Op is treatment plan that seeks to prevent disease, promote health, enhance the quality of life and to maintain or prevent deterioration and degeneration. The plan begins following the stabilization of an acute or chronic condition, in the absence of any acute or chronic complaint or when active care is no longer expected to improve the condition. This is accomplished through monthly check ups and bi monthly treatments.

Membership includes:

Up to 3 treatments per month of  Chiropractic Manipulative Therapy, Soft Tissue Therapy,  or laser therapy (one unit each) plus you can use  intersegmental traction ( the roller table) on your visit ( optional.)

Should a member experience any condition, complaint or complication that would require additional treatment on an acute basis, the appropriate referrals and adjustments to your plan will be made if needed and the membership will be suspended until your the condition is stabilized and the patient is again a candidate for maintenance care. 

Enrolees agree that in no future time will a request be made asking  the office of Dr. David DeFries D.C.  to submit a claim to a third party for services performed as part of the maintenance program. 

Patients also agree to report any accident, injury or change in condition that may require additional care and/or a change in financial responsability ( a car accident, work injury or slip and fall)

Patients agree and understant that by signing this agreement they are purchasing the above noted services in advance and should they not use the three visits in any thirty day period that they are forefitted and no refubds will be issued. Patients are free to cancel the membership before seven days from the beginning of the next months start date. Cancelling membership does not entitle a refund for the present or prior months fees or the initial enrollment fee. 

All new patients or patients who have not presented in the office in the last three months will require an examination to determine if they are candidates for maintenance care. The fee for the examination is $60. Patients who have presented for care within the last three months and do not have any new complaints or worsening of prior complaints and who are presently candidates for maintenance care are exempt from this examination fee.

Upon enrollment,  an administration fee of $40 will be collected along with the first months payment of $60. The administration fee is per household and any additional household members may join  provided that following examination they are maintenance therapy candidates. The administrative fee will only be waived once per household member and should the member(s) cancel their membership and then desire to rejoin the program, they will be required to pay the administrative fee and examination fee again. Any member who cancels and rejoins more than twice in one calendar year or otherwise fails to honor the good nature of this program may be denied membership.

Thereafter, the membership fee(s) will be charged to the card on file once a month.
It can't get any less painful to stay out of pain!
